医保支付方式改革是激励医疗机构控制成本、提高医疗服务质量的内生动力,也是优化医疗资源配置的重要杠杆。在全球医药卫生体制改革中,筹资、支付、组织、规制和行为已经成为世界各国医药卫生体制改革的关键性抓手。尤其是医保支付方式,从20世纪70年代开始,成为世界各国医药卫生体制改革的重要内容。
Reforming the way health care is paid is an in-house incentive for health-care institutions to control costs and improve the quality of health services, as well as an important lever for optimizing the allocation of health-care resources. In the context of global health-care reform, financing, payment, organization, regulation, and behaviour have become key players in health-care reform around the world.
目前,我国基本医疗保险覆盖率已经达到95%以上,医保机构成为医疗服务的最大购买方。医保支付方式改革对于保障和促进全民健康,实现健康中国战略目标具有重要意义。
At present, our basic health insurance coverage has reached more than 95%, and health-care institutions have become the largest purchasers of medical services. Reforms in the way health-care payments are paid are important in order to guarantee and promote health care for all and to achieve the strategic objectives of a healthy China.
2017年6月,国务院办公厅印发的《关于进一步深化基本医疗保险支付方式改革的指导意见》明确支付方式改革的主要内容:实行多元复合式医保支付方式,重点推行按病种付费,开展按疾病诊断相关分组付费试点,完善按人头付费、按床日付费等支付方式,强化医保对医疗行为的监管。
In June 2017, the Office of the State Council issued a Guidance for the Further Development of the Reform of the Method of Payment of Basic Health Insurance, which identifies the main elements of the reform of payment modalities: the introduction of multiple and complex health insurance payments, with a focus on the introduction of patient-based payments, the implementation of a pilot fee-related sub-groups for disease diagnosis, the improvement of the payment-based and bed-day payments, etc., and the strengthening of health-care supervision of health-care behaviour.
疾病诊断相关分组(DRG)是用于衡量医疗服务质量和效率,以及进行医保支付的一个重要工具,其实质上是一种病例组合分类方案,即根据年龄、疾病诊断、合并症、并发症、治疗方式、病症严重程度及转归和资源消耗等因素,将患者分入若干诊断组进行管理的体系。
